If a child is being transferred from another Orange County Public School, the only documentation needed is proof of address.
The following items are needed to register a child new to Orange County. Please make sure there are no documents missing, and complete the registration packet.
Physical, within one year, Birth Certificate (State Certified, no hospital certificates)
Proof of Immunization (680 A Form) should have the following:

DTP Series Polio Series

2 MMR (Mumps, Measles, Rubella)

HIB
-
Hepatitis B Series (mandatory for KG, 1st, 2nd, 3rd and 4th grades)

Varicella (Chickenpox vaccine date or date child had the illness is mandatory for PK, KG and 1st graders)
Current Proof of residence - You will need one of the following:
Report Card showing last grade completed
If person enrolling the child is not the parent, documentation showing legal guardianship is required.
Incomplete registrations will not be accepted. |
